agree with dan re: one on one nursing. this was a requirement in the published trials and i know of NO hospital in the NY area which can provide one on one nursing in a crowded labor suite. without satisfying this requirement, IA would result in a risk manager's nightmare. with regard to who NEEDS monitoring, besides VBACS, all receiving oxytocin should have EFM since the dublin trial showed reduced seizures in the monitored group. the FAT, altho promising, has not been studied in a randomized trial - perhaps it should be. finally, when barry shiffrin was asked who needs EFM, his reply was "all those who are gonna have problems." since we rarely don't know ahead of time who those patients will be, i leave to your discretion how to proceed.
