PTE prophylaxis

Hi everybody. A little doubt: do you currently use dicoumarinics in low doses for post surgery pulmonary tromboembolism prophylaxis in addition to heparin 5,000 U/12 hs 2-12 hours before intervention (according to the pt risk factors) or do you just use heparin alone? Many thanks.
