(1) In my copy, the important pages labelled "Protocol for..." are blank, although all the introductory/epidemiological data are intact.

(2) After a virus was distributed through this list a few weeks ago, I believe we agreed not to send enclosures. The risk of virus transmission is much higher, and it usually introduces all sorts of software-compatibility problems. (E.g., yours was binhexed, which is fine for us superior Mac users, but often causes fits for PCs.) Better to chop up the document into several standard e-mails.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
