I'm on vacation, on AOL, and I can't see the text Geff wrote to edit; sorry.

Anyway, If someone is a known carrier, and even if treated, I would still treat her in labor, and find no reason to ever culture her again.

Geff, if someone who is culture negative is ruptured for 18 hours, I would re-culture then, and treat, because this is a high risk situation for GBS passage, and we all know that some of the people who are negative at the 36 week culture are positive later.

Someone said that they do not culture those who are having scheduled sections; I disagree, and would culture them in case they present in labor and elect to deliver vaginally, or don't have time for a section but do have time for antibiotics.

A GBS question for all:

In a carrier who is scheduled for a section at 39 weeks for a known breech, and assuming you normally use a single dose of a first generation cephalosporin for routine prophylaxis, what and when do you use for this patient?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
