Don't know the origin but it's very British. The wooden spoon is a figurative trophy due to who ever comes last in any contest. For instance the nation who comes last in our annual five nations rugby tournament ( England, Wales, Scotland, Ireland & France ).

In the foregoing bit of Cochranes criticism he talks about which specialities he thought did babdly at EBM ( he was writing ~20yrs ago and before it was called EBM ! ). He reckoned surgery, psychiatry and cardiology were pretty bad but ( then ) obstetrics came bottom.
