Re: Placenta IncretaFrom: ATFSOCCER@aol.comSat Jun 21 08:40:53 1997
In a message dated 97-06-21 04:11:18 EDT, you write: << NOW---If the pack is sucessful in the short term (hematocrit is stable tomorrow, fundus is firm, no excessive bleeding) WHEN O WHEN should I pull the pack? She realizes the packing is a last resort kind of thing, and she realizes that another pregnancy would probably yield a simolar complication, but she really does not want to have a TAH. Advice? Countenance? Prayers? THANX!!!!! >> My anecdotal based suggestions would be to: 1. Stabilize , foley catheter, a 2nd IV 16 or 18 guage and 4u blood available. 2. Broad spectrum IV antibiotics 24-48 hrs. 3. Pull pack in 24 hrs. 4. If stable wait 24hr and discharge with methergine. Consider hysteroscopy with resectoscope in 6 weeks to remove any placental tissue?? 5. For intractable bleeding I would consider consult with radiologist for uterine artery embolization.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
