So far I have seen only one suggestion that included steroids, and that was to inject them (along with lidocane) in the area of the pudendal nerve. How about a local injection (in the area of the pain) of a combination of steroid (eg depo medrol) and lidocane?

Secondly does she have any evidence of a stitch abscess? I have removed a few of these from old episiotomy sites with good results. In some of these cases the patient presented years after the fact. Usually there is a small nodular area palpable .
