Re: Ablation for a 14 week fibroid, I personally would not recommend it as the chance of success is low. I further have never seen or read a report of a large fiboid uterus "shrinking" significantly after a submucous resection, followed by endometrial resection/ablation. If abdominal surgery is too risky for this patient, Lupron could be given for 2-3 months, and if the fibroids are not calcified, you might get a 50% reduction in size thus achieving a therpeutic window for resection/ablation. Resection of very large fibroids require a fair amount of operating time, so do keep close watch on I&O. This latter problem would probably be lessened by use of a Vaportrode electrode as opposed to a conventional loop electrode, as the loop will predispose to greater fluid absorption, and the Vaportrode, theoretically at least, will tend to seal the venous channels.
