Ablation and 14 week fibroid uterusFrom: ENDODOK@aol.comSun Sep 29 11:26:56 1996
Re: Ablation for a 14 week fibroid, I personally would not recommend it as the chance of success is low. I further have never seen or read a report of a large fiboid uterus "shrinking" significantly after a submucous resection, followed by endometrial resection/ablation. If abdominal surgery is too risky for this patient, Lupron could be given for 2-3 months, and if the fibroids are not calcified, you might get a 50% reduction in size thus achieving a therpeutic window for resection/ablation. Resection of very large fibroids require a fair amount of operating time, so do keep close watch on I&O. This latter problem would probably be lessened by use of a Vaportrode electrode as opposed to a conventional loop electrode, as the loop will predispose to greater fluid absorption, and the Vaportrode, theoretically at least, will tend to seal the venous channels. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
