Major adhesions at ime of C/S do seem to be a very rare but difficult problem. Recently one of our senior consultants ended up going through both the anterior and posterior bladder walls to deliver a young prim breech at term. She'd had a laparotomy for abdominal trauma in childhhood but no problems since. At op no route could be found to the lower segment except through the bladder ( the initial opening wasn't intended!). Last week I saw a urologist demonstrating bladder reconstruction (Clam cystoplasty) for severe detrusor instability and commented that these women sometimes end up having a C/S but they'd never been involved in such a case. How do you avoid the bladder/bowel?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
