Dear Listers, I am a CNM doing a Masters and have a Primary Care rotation this semester. As I have not had any experience with HIV + individuals, I chose to do my clinical at our local HIV primary care clinic. We have mandatory HIV testing of pregnant women in Texas, and two days ago we received our first positive post partum woman in the HIV clinic. This is a 17 year old primipara now 4 weeks PP. Her OB referred her to the Regional Perinatal Center in Galveston (350 miles away) at 12 weeks gestation, where she stayed until delivery. Although I have not seen her record, she tells me she experienced a normal pregnancy in every way, except for her HIV status. It seems she was referred for AZT therapy only. Like many physicians, the ones in our area (South Texas/Mexico border) seem reluctant to care for HIV positive clients. However, it seems unethical to me send this girl 350 miles away for AZT. Am I missing something? Is there some complicated trick to managing AZT therapy in pregnancy? My clinical site would have been glad to manage this area of her pregnancy, perhaps it is a matter of mis-information. What do you think? Elizabeth Davidson CMM Brownsville, Texas

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
