I have an asymptomatic patient with a 4 cm fibroid which has a necrotic center. Last year the fibroid was 3 cm with just a hint of central necrosis. She is 42 years old. Is central necrosis enough of an indicator of sarcomatous degeneration to warrant hysterectomy in an asymptomatic patient?

Necrotic ????? I doubt that she has had necrotic tissue in her uterus for over a year. This may be the long term results of necrosis, but I'll bet that if aspirated the fluid will be clear and not necrotic. This sounds like cystic degeneration of a leiomyoma to me. The likelihood of sarcoma is certainly lowl. Is it high enough to make a hyst indicated?? Good question, probably lots of answers. If it is an indication, it probably was one a year ago also.
