Bite the bullet. Do an intentional cystotomy and cannulate the ureters at the beginning of the case. Get the base of the bladder off the lower uterine segment while you are looking thorough your cytostomy. It seems I end up doing this about once a year and I have never regretted it. - Kevin Ault, MD University of Iowa
