I had a patient last year, G2,P1, s/p c/s, who presented for a second section because of a t-d scar in her first cesearean. At her second section she had such thick,dense, vascular, adhesions of anterior wall to fundus that we had difficulty getting to the lower uterine segment. The patient lost a fair amount of blood during the case but did well postoperatively. She and her husband want a third child and want to know if there is anything that can be done to minimize her risk at a subsequent c/section. I didn't have an answer but wondered out loud if pelviscopy with lysis of adhesions and possibly interceed would lessen the adhesions and improve her outcome. Anyone with a similar experience or thoughts?
