PIH ManagementFrom: CNMPAT@aol.comWed Sep 18 20:14:51 1996
<<<<For those who begin with the initiation of induction, I wonder if they use PGE gel, or other cervical enhancement techniques and if they do, do they consider that part of the induction period and therefore cover with Mg++, during the cervical ripening period. >> When I have a woman needing delivery with PIH, I start with PGE gel, and many times this will get her going in labor (we use Cervidil). If I am concerned about her B/P, I keep her in the hospital. We are a small community hospital. Some folks send them home and have them return in the AM for pitocin. I wait to start Mag once the contraction pattern starts. I recently had a case like this. I kept her overnight and did the Certidil. She started having some contractions, and actually her B/P looked good in the morning, but I went ahead and started the Mag too. She became quite sick during labor, but the birth went well (at 38 weeks). How long AFTER the birth do you folks keep the Mag going. Some folks say 12 hours, some 24 hours. What seems most reasonable, based on your experience ? The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
