One large HMO in the New York City Metropolitan area (and along the eastern coast of the US...>2M members) has been reimbursing for some time now for time and effort ---> lowest global reimbursement (without add-ons) is elective repeat cesarean; highest global reimbursement (again, without add-ons such as a high-risk component) is for successful VBAC.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
