I appreciate everyone's help. Diane Peterson suggests that the problem is due to bartholinitis, and her description is right on. The pain my patient describes is not the usual "vulvodynia" presentation. It begins with arousal of any type, and can last for 2 days, in her case. There is no lesion or evidence of obstruction on exam.
