I have a 37 year old patient who has had Norplant for 3 and a half years. She noted breast tenderness, then missed her expected period. A uhCG was positive, and the exam consistent with dates. Transvaginal sonogram showed twin gestations, clearly in separate sacs. The published failure rate for Norplant is 2 per thousand per year, and when adjusted for weight (this patient is 125 lbs) perhaps slightly lower. Altough that use of Norplant has dropped significantly in our practice, I cannot remember a failure in the past five years. The presence of twins seems to be incredibly rare. I just performed a Medline search and cannot find any mention of twins. I have contacted the FDA, mostly at the patient's request, as she is concerned about the possibility of a bad batch of Norplant-fortunately we have always recorded the lot numbers. I am waiting for follow up from the FDA. Has anyone seen twins as a result result of Norplant failure? Paul D. The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
