How would you manage labor and delivery plans for the following patient? 36 year old G3P2 with 2 previous C/sections in Mexico in 1984 and 1986 for average sized babies. Patient does not remember why first section was performed, but reports it was done "in a hurry". Second section was a planned repeat. Surgical records from the sections are not available, but the skin incision is vertical. Would you offer this patient a trial of labor? If so, and PROM with light meconium at term was followed by widely spaced contractions and non-progressive labor after many hours, would you offer Pitocin augmentation? If meconium became thicker and/or variable decelerations ensued would you do an amnioinfusion? Just trying to get a general sense of the way others would handle this type of situation. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
