I have always had the impression that estrogen had a vasodilative effect on the vessels of reproduction and that catacolomines had a vasoconstricting effect, also that progesterone alone had a constricting effect (Resnik R; The endocrine regulation of uterine blood flow in the nonpregnant uterus: A Review; Am J. Obstet. Gynecol, 1981; 140:151-156). What is the effect of progesterone on uterine perfusion? Or is there a percent estrogen/progesterone which moderates perfusion? Can it be that it is the lack of perfusion which causes some recurrent spontaneous abortions? I think so, especially were stress and catacolomines are a concern. We developed an argument for this in our publication, but it was really searching literature to confirm what we were seeing. See: DuBose, Hill, Cunyus, et al; Sonography of Menstrual Changes in Arcuate Uterine Blood Vessel Diameters; J Cardiovascular Ultrasonography, 1985; 4:243-251. This info is dated now, the research was done before the advent of endovaginal transducers. However, it seems our observations of changes in uterine vessel diameters throughout menstrual cycles might be related. Peace, Terry J. <<No one seems to be able to provide me with any data to support this practice but this gives both the physician and the patient that something is being done to prevent miscarriage. In my opinion, the only patients who may benefit from this would be those with recurrent spontaneous abortions and have documented luteal phase defect.

I don't think we know the science behind it, probably because the causes of "bleeding in the first trimester" are multiple.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
