If all conservative measures have failed ( as you so indicated) and the patients Sx. are truly manifestations of PMS, my experience would suggest that the result of the BSO would be very positive improvement. It sounds radical, but if nothing else helps, surgery may be the answer. Unfortunately, in the managed care setting, I would guess that approval will not be forthcoming!!
