?Rx for lactating pt with ovarian failure

I would like to ask list members for advice in treating a patient with ovarian failure who is lactating. She was on HRT and conceived with IVF; pregnancy outcome was a viable male infant. She is currently on no hormone replacement. My concerns now relate to the return of menopausal symptoms, increased risk of bone loss while lactating, and exposure of the infant to exogenous estrogen.

Any helpful advice would be appreciated.
