Re: 20 Week Fibroid Uterus

I was completely aware that the uterus would regrow when I attempted various types of "Medical Menopause" therapy but it was all the patient would accept. Regarding Dr. Suarez's concerns regarding cancer; I too share those concerns but it is not my idea leave this mass within her pelvis, it is hers. She is not amenable to surgery unless I could prove that the uterus was malignant. This would require its removal which she does not want. I know an endometrial biopsy would not diagnose a leiomyosarcoma unless it had eroded into the endometrial cavity. I have actually diagnosed two of these cancers with endometrial biopsies, but that was not my hope. I felt I would look pretty foolish if I simply assumed she had fibroids and not an endometrial carcinoma which I could easily rule out. At least that type of cancer is now off the list.

Any thoughts as to whether or not an MRI of the pelvis or Uterus would be helpful since its advocates feel it can be used to stage endometrial cancer. One of the leiomyosarcomas I diagnosed with an endometrial biopsy also had an MRI. The radiologist immediately called me to ask about this funny looking uterus. He thought it looked malignant on the MRI.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
