regarding longterm heparin management, we would keep her at 2-2.5 times control, stop heparin in labor and resume afterwards, and continue through 4 weeks post-partum (arbitrarily chosen #) - and for our OB's on long term bed rest, we use intermittent pneumatic leg compression
