There is not much in the way of randomized trials in this situation, but for what its worth, I would keep her on IV heparin at least 10 days in the hospital, then switch to therapeutic SQ heparin (usually needs 3 injections/day), check heparin level if you can (or PTT if you can't) midway between doses. The level should be therapeutic not prophylactic.

She also needs home health aide or something so she can stay at rest until her clots resolve to prevent recurrent embolism.
