I came across a missing adnexa once at an attempt at a BTL (she ultimately had a 'UTL'). The woman later told us (on astute medical cross examination) that her mother was missing one side and one kidney. We ordered an IVP on the patient, but I don't remember whether she had only one kidney or not.

We figured it was an embryologic accident which caused the entire mesonephric ridge to fail to develop, and it must have had some familial component, since the mother had a similar condition. I don't think we ever found that precise terminology in print, though.
