The "all fours maneuver" does many things. It changes the direction of gravity (and to some extent the long axis of the fetus), it does alter the outlet somewhat but it doesn't seem that it would a great deal, it does tend to disimpact the posterior shoulder (gravity again), but it is probably simply the act of twisting and turning the mothers torso to get into the all fours position that tends to rotate and or disimpact the shoulders.

I don't believe I've seen it described in the obstetric literature though it has been taught by and to midwives extensively, and is almost certainly in the midwifery literature.
