I believe the "all fours" maneuver, as you describe it; the knee chest position, as practiced by many, or the McRobert's maneuver, as described by Bernard Gonik about a decade ago, all "do" essentially the same thing, except that those maneuvers that rotate the mother so that her abdomen faces the floor make the posterior arm more easy to manipulate.
