Endometrial Bx, etc

Excuse me for stepping in a little late to this conversation, but I just have to put in my .02. I don't do a lot of endometrial biopsies anymore (I am a fellow in MFM), but I do do a lot of 1st trimester terminations (moonlighting for Planned Parenthood). In my experience, 95% of women have *a lot* of bad cramping when you place a tenaculum on the cervix. I started using 1-3 cc of lidocaine in the anterior lip of the cx before I place the tenaculum. Most women do not feel the lidocaine going in, and if they do it is a very mild burning sensation. Then when you place the tenaculum they dont feel a thing. So my feeling is, if you can do the EMB without a tenaculum, great! If not, by all means use some lidocaine before the tenaculum.
