There is no art in VBAC, its basically a crap shoot. Even if you do a stat section within 5 minutes of signs of a ruptured uterus you are not gaurantied a good outcome. Durring the consent process you are obligated to inform the mom that even with the best precautions you are exposing the baby to greater risk than a planed c-section, not to mention a failed VBAC may carry more morbidity for the Mom than an elective planed repeat c-section. I still do VBAC's and make sure the above has been documented in the chart, though it may not matter much in case of a bad outcome.
