Many decades ago Bill Keetle,chair at Iowa, did a research paper. He found that hospitals doing only 600 deliveries a year either lost monry or were unsafe. Less than 600 yhe hospital was both unsafe and a money loser. That is why Iowa developed a large referral center and maternal transport system. Thus you and the hospital are in the deep stuff..Unsafe at any speed and a money loser. Something must give.. 2 part question.

1. Do you believe it is required to have an ultrasound immediately available in a Labor and Delivery unit? I work in a small rural hospital. We do around 100 deliveries a year. Hospital has an Ultrasound unit in the radiology dept. However, it takes a good 5 minutes to boot up and its a large machine and is not really a good machine to transer to the OB unit during an emergency. I believe that we need a "portable" type machine in the L&D unit. In my residency we called this machine "Rover" and and can recall using it countless times.

2. How do I put pressure on my administration to purchase this item? I have been asking now for a good year. They pay lip service to looking for an affordable item but after a year of this and no ultrasound, its becoming ridiculous. Of importance, I am employed by this hospital and its the only hospital that I practice.
