amniocentesis and fetal malformations

I need Your experience and opinion about what happened to my Collegue in Our Department of Obstetrics.The birth at forthy weeks of a baby with some malformations: 1)Agenesia (absence) of four fingers with hypoplasia of thumb of right hand.2)hypoplasia of four fingers with normal thumb of left hand;3)right foot agenesia (absence)with hypoplasia of tibial bone;normal left leg. She have done amniocentesis in another Obstetrical Department at about 18 weeks (normal results) and had a hospitalization in another (again)Obstetric Department for abdominal pain at about 23 weeks. She has done several echographies (by my Collegue,the other Doctor before amniocentesis,and by another Collegue during recovery for abdominal pain) and nobody has noted the problems.Italian echographic protocols say that after 22 weeks of gestation a Doctor is obliged to see only fetal growth and presentation. What happened and presumably at what gestational week happened? In Italy legal therapeutical abortion is not allowed after 23 weeks of gestation. 1)amniotic bands(bridles) not evidenced by echographies? 2)blood fetal embolism? 3)consequence of amniocentesis (infection)? 4)other?
