Sorry to disagree, but a failed vacuum is always worth an attempt with forceps unless you were really not sure, as you began, that you were doing the right thing - and that should never be the case.

My experience is that if Vacuum doesn't work, it's never the vector or the application, it's always the indication :-)-O.

I don't have evidence but I was told that a failed vacuum means a C/S and *NEVER* a forceps.
