I have a copy of the one AAFP was producing for awhile. They don't sell them anymore, so I don't think it's a problem to use them or modify them. I can scan it and send it to you as a pdf when I get a few minutes. I can't find my English version at the moment, but I assume you or someone at your clinic reads Spanish. We still use this one at our outreach clinic and it seems to work well.
