48 yo with a history of adenocarcinoma in situ of the cervix 10 years ago, treated with LEEP x 2 and q6 month Pap for 5 years, presented for yearly exam. Vulvar exam showed a 2cm lesion. Punch biopsy shows VIN3. Pap pending. I need to bring her in for a more complete vulvar exam, but if this is the only lesion, would you simply do a wide excision or should she be referred to onc for more aggressive surgery? thanks, Glen

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
