I think head above the brim is a great rule, unfortunately it becomes not very usefull when BMI>45, seems about 1/3 of the MOM's I deliever have a BMI of >45 these days. One of the reason's I like Tuucker/Macleans is the shanks are about 18 inches long and the blades are non-fenestrated. There are only one pair of non-fenestrated Kielland in the D.R. the shanks are only 14? inches long, when Mom's BMI is > 45 the head may be at +4 and you will not be able to apply them because of soft tissue dystocia.
