Emilio, I have followed this routine: first, the woman is in all- fours, with arms and legs both at 90 degree angles to her body. If that isn't enough to dislodge the shoulder, then ask her to bring one knee forward. In this position, both palms on the bed and both knees on the bed, but with one knee placed about 50 cm in front of the other, which is placed directly beneath the hip. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
