Emilio, I have followed this routine: first, the woman is in all- fours, with arms and legs both at 90 degree angles to her body. If that isn't enough to dislodge the shoulder, then ask her to bring one knee forward. In this position, both palms on the bed and both knees on the bed, but with one knee placed about 50 cm in front of the other, which is placed directly beneath the hip.

I think the way that Susan describes would work, too, but for some women, it might not feel as stable as the way I describe above.
