Based on what do you think doing a little twist would damage the myometrium? And even if it does, the myometrium is so resilient, it will bounce back, it will heal. In fact a patient will have to twist her pelvis anyway in order to get on her all 4's. I am proposing doing the twist and then get back into the McRoberts position and push the baby out. And if you think that's stressful to the fetus - I wonder what to fetus have to say to the intravaginal orthopedic surgery. (But seriously, I am glad you were able to get your baby out by cutting its clavicle.)

If you really think about it - rotating the mother hips may actually not be so physiologically dissimilar to the Wood Screw or Rubin's Maneuvers.
