I use the EndoPouch (Ethicon). Admittedly it's an occasional nuissance to have to chop up the adnexa a bit before it can be extracted in the bag. Completely worth the trouble however. I use the Endobag. I find myself rarely needing to extend the incision to remove a normal sized ovary. I leave the abdomen inflated and pull the port out with the bag. If the bag does not want to come out, I open the bag and grasp the ovary with a pair of Allis clamps and pull everything out together. If there is an ovarain cyst, I would puncture the cyst is the bag and keep everything contained in the bag before I pull it out of the abdomen. Once in a while, I would find it necessary to extend the incision. But I prefer that over making a new incision in the vagina and converting a clean procedure to a "clean contaminated" procedure. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
