Because if he doesn't, he will get splattered in the face with blood as the CO2 percolates out of the incision in the vaginal vault. OTOH, it is certainly easier and safer to make the incision from below if the "coast is clear" above, ie with pelvic and abdominal cavity filled with gas, and vital structures out of the way. Or one could make the incision from above (which is what I would do if I were to utilize that technique).

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
