I had one of these partial inversions not long ago... patient became shocky. midwife pointed out that the uterus was well clamped down there was in fact very little bleeding,by the time I arrived and you could feel still what felt like a firm ...very firm..."small" uterus above the brim...no dent to feel in the top.I did not pick up on the fact that the uterine body that I was feeling was far too small dimensionally for an immediate post partum ut...like they called me , and I got there within 15 minutes and when I examined PV I could feel a mass , which on first thoughts becuae of what I thought was the uterus above the brim, I wondered if it might be a prolapsing fibroid...then I put the facts together..small ut body..mass in vagina..shock ..= uterine inversion. it took me a few minutes to"clue in"maybe 3 minutes..(dumb I guess)...maybe less , it just felt long. very difficult to reduce...probably becuase it had been now 20 mins since delivery..and midwife had given oxy and ergot which we still had... all hell to reduce even under general anaesthetic...nitro etc...but eventually got it .. horrid... and I felt quite dumb for not realizing that an immediate PP uterus is not 2-3 fingers above the brim regardless of how much oxy etc... hopefully it will stick in my aging mind if that partial happens again..I think it will as it was all rather horrific..and the woman was in fairly intractable shock frances wren
