Welcome to Formedic's charting revolution!

Simple, efficient and thorough, Formedic's Medical History Questionnaire (MHQ) will quickly become an indispensable way for you and your staff to manage patient visits.

MHQ is a new and exciting free service that gathers the patient's medical history and symptomatology prior to the physician encounter. Start saving time and money with MHQ today!

Document Better

MHQ organizes the patient's medical history into familiar, clinical terminology and generates a pre-filled patient record form on which the entire patient visit can be charted. MHQ delivers the thorough, already-documented subjective medical history you need to effectively engage your patients.

Saves Time, And It's Easy To Use

MHQ saves you, on average, 4 minutes per patient. That's over an hour of additional time each day. With MHQ charting is organized, quick and simple!

Reduces Costs, And It's Free

You'll think you hired an extra assistant - with no cost to you. And, as with all Formedic resources, MHQ is available to healthcare professionals completely free of charge.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
