Most of the available labs hereabouts will set it up as a reflex for ASCUS or CIN if asked. However, if its reflex, it does require, IMO, discussion with the patient at the time of pap acquisition. Also, it has different cost implications for folks based on insurance plans, self pay status, etc. And beyond that, if I read the last ACOG selected journal article summary and commentary that comes with their periodic mailings, the data still conflict as to whether lives are saved or significant lesions missed at significant negative impact on the patient with reflex HPV screening. I think being selective, considering a patient's past hx of compliance (or following your gut instincts on her reliability to f/u), and/or repeating ASCUS smear with an interval smear is still a reasonable approach and one many patients will select...but doing it with an appropriate informed discussion is a killer, for time and bottom line. Then again, we do want to keep those ledger lines filled for our friendly labs back offices.
