Breakthrough bleeding is usually due to atrophy after months on progesterone. Thus it would be better to increase the oestrogen or decrease the progestogen, which is the opposite of what you are suggesting. I think you will fail. Better to suggest Brevinor.

20 YO P0 on OCPs for cycle control (irregular, up to 60 day cycles) is taking Yasmin 42 days on/7 off, and has been successful on that for around 2 years. Previously, she had been tried on Mircette and Levlen, but had breakthrough on those. Also germane is that she has migraines for which she needs Imitrex from time to time.

I am thinking about suggesting either LoOvral or O/N 1/35, and can't decide whether to recommend a normal 21/7 regiment, versus the 42/7.

Any thoughts about which pill/how to take are most welcome.
