I would just repeat NST in 48 hrs, if she was 37 wks with favorable cervix I would probably induce. Most likely nucal cord or baby grabing on to cord. Why induce at all, there is always the possiblity of a true knot forming or nuchal cord that you can't see on U/S, if the baby is still born with a nucual cord there is that nice record of the cord compression. Thank goodness you have a good indication for the NST.
