Know the study well, not all the well done ("retrospective, it was neither blinded nor randomized. Decisions regarding duration of neonatal hospitalization, need for treatment, and neonatal abstinence score are generally considered to be objective, but clinicians were aware of maternal drug use and methadone dosage"). It was a retrospective study that looked back over 11 yrs. There was also a study in 2003 done by Berghella, et al revealed the opposite findings of Dashe's study. Also, more interesting is the fact that in a study that looked at whether cord blood concentrations of methadone predict the severity of NAS (Archives of Disease in Childhood Fetal and Neonatal Edition 2004;89:F390-F393) it was found that maternal methadone dose did not predict the need for treatment. However, infants who required treatment had significantly lower methadone concentrations in cord blood than the group who did not receive treatment.
 
I have a theory about that if anyone is interested!

There is a good article that addresses it in the Green Journal, v100, #6, 12/02, Jodi Dashe, et.al, “Relationship between maternal methadone dosage and neonatal withdrawal.  It can occur on as low as 20mg/d (12% of mothers) and increases to 90% on mothers receiving 40mg/d or more.  It’s almost a linear correlation and also correlates to duration of neonatal hospitalization.

Since you're "into" Methadone and pregnancy, what's the rough upper limit of maternal dose of Methadone that you can use WITHOUT getting withdrawal symptoms in the neonate?
