Sharon- welcome. We recently treated a patient admitted to us who was taking 180mg/day of methadone as prescribed by a county methadone maintenance program. We discussed decreasing the dosage prior to her scheduled delivery for neonatal benefit, but she threatened to leave AMA. I understand all patients are different, but do you have general advice for decreasing high-dose methadone prior to delivery to decrease neonatal withdrawl symptoms, or is this even advised? Thanks very much.
