A small percentage of moms (it varies by ethnicity) rapidly metabolize codeine to morphine. I wouldn't say codeine is contraindicated in breastfeeding moms, but needs to be given for only a few days, and if pain continues, the situation needs to be re-evaluated, which is why I have to keep reminding my residents not to send more than 2 or 3 days worth of T3 home with breastfeeding moms and to tell them to call if still having pain requiring narcotics when that runs out. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
