Have always recomened fiber, even when it was not fashionable. The softer the stool the less likely it will stress the suture line in a 4th degree, offcourse its more likely with high fiber the patient will be incontinet of gas but there is nothing you can do about that once the damage has been done. Having hard stool sitting on a suture line is probably the reciepe for a breakdown. IMHO true 4th degree tears are extremly rare I don't think I have repaired more than a half dozen. 3rd degree tears are not rare and I have repaired dozens.
