The difference may be "routine" - ALWAYS doing AROM at ... say 3 or 6 cm vs labor is going (and going and going) and it is done therapeutically - so hopefully the "practice" would be "judicious" use of amniotomy as opposed to "routine" - and although I have not read this entire Cochrane review - it does not sound like it separates between the judicious to augment vs routine of "we always do it at 5 cm" ...

Just my $0.02 - could be wrong AND I don't do Ob anymore so should probably not even give those 2 cents worth!
