Most likely both serum screen and doppler reflect placental disease. Would still consider karyotyping since abnormal karyotype can manifest with abnormal placenta. Additionally, doppler of the ductus venosus, which hopefully is normal, could be followed. Is Larry Platt lurking here lately? Allan Bombard?
